Fic Search: Sam/Jack

I have no idea when or where I read this fic, but I do know that it isn't recent, it's at least a couple of years old I think.
NB: Significant story spoilers under the cut.
SG1 were on a mission, something happened and Sam died. I think Jack quit the SGC. However, the bit that I do remember is that Jack spent the better part of the following year talking to Carter's ghost. And Carter's ghost talked back. And they have perfectly sensible conversations. I'm pretty sure that Jack realises that talking to a ghost is Not A Good Thing, but if I remember correctly, he believes that the Carter he's talking to is a figment of his imagination and he can hold a conversation with her because he knew her so well. It turns out that Sam isn't really dead, she's been in a coma (or something) on the alien planet where she 'died' and she really has been talking to Jack for the past 12 months. There was a reason why she couldn't tell him straight up that she was alive, I think it had to do with the conditions she agreed to to be able to talk to Jack, or there was some sort of mis-communication between her and Jack, or Jack really thought he was going crazy.
I want to say that I think the aliens didn't want to let her go, but they were giving her a fighting chance to be rescued although they weren't going to make it easy for SG1 to rescue her, i.e. she couldn't directly tell them/Jack what was happening.
ETA: I'm pretty sure there's a car journey as well. Jack's in his car going somewhere (Minnesota, I suspect) and ghost!Sam joins him.
If anyone has any idea what this fic is, I'd be very grateful.
